GOT PLANS: ADVANCE CARE PLANNING WORKSHOP CAN HELP YOU BE PREPARED

Do you have a health care power of attorney? Do you have a living will? Do your loved ones know how to speak for you if you are ever unable to make your own medical decisions? It's important to have discussions with family, friends and health care providers and to execute written advance directives in accordance with North Carolina laws.

If you want to put a plan in place but don’t know where to start, Forsyth County Central Library is hosting an Advance Care Planning Workshop on Wednesday, Jan. 31 from 1-2:30 p.m.

A Trellis Support Care representative will discuss these topics and provide the documents for you to assign a health care power of attorney and create a living will. They will also provide guidance for filling out these forms. Registration is encouraged but not required.
